This form is used within the Photography and Video teams to schedule your shoot and get details for what you are requesting. Requests are due in to the director of Administration and Communications at least 3 weeks prior to your event or shoot date.

      PHOTO: Feel free to give suggestions and let us know if you have intentions of using photos for anything in particular once we are done. (For example, putting them vertically in a christmas card which would mean we should shoot portrait vs. landscape, if you'd like a portrait with a backdrop or candids, etc.) VIDEO: If this footage will be used in a future video promotion please also fill out a "Video Promotion Request Form" and turn it into Justin Isacson, the Video Ministry Director.
